  1. Book your New Client Intake appointment in my convenient online system. You can create a client account and fill out the online Intake form. Allow for 15-20 minutes to complete the questions. If you prefer, you may use this PDF New Client form and scan and return it to me. Kindly complete the questionnaire 48 hours in advance of our meeting so I can review in advance and start formulating your plan. This helps maximize the value of our time together.
  2. Please also download, print, and complete at least a 3-day Food Journal. Please be completely honest! Recording two workday or weekdays of food and one weekend day is helpful. You can bring this to our first meeting.
  3. Please familiarize yourself with my Policies and Consent form. You can bring a signed version to our first meeting or complete it with me.
