NEW CLIENTS: SCHEDULE EASILY

  1. New Client Intake: As soon as your Intake meeting is scheduled, you will receive an email with comprehensive instructions including the link to the New Client Intake Questionnaire. Please plan on spending at least 15-20 minutes filling out the Intake questionnaire. You can save answers and go back and edit as needed. Or, use this PDF New Client Intake form, scan or photograph the pages and email to info@deliciousnutritioushealth.com. In either case, please complete and/or send your answers ahead of time so that we can maximize the value of our time together. Complete the Intake questionnaire three full business days (72 hours) Monday-Friday in advance of our meeting so I can review and start formulating your plan. We may reschedule your meeting if your Intake questionnaire does not arrive in time for me to prep. For example, if we are meeting on a Tuesday, complete the questionnaire by Thursday. My goal is to assemble or send any necessary handouts, questionnaires or links you may need in advance of our interactive discussion! If you have not been feeling well for a long time, creating a timeline of when major symptoms began or when you were injured, hospitalized other otherwise treated would be very helpful!

  2. Food Journal: Please record what and when you eat and drink for at least 3 days. Ideally, record two days of regular weekday eating and one day of weekend eating. If you are tracking your food online or keep a journal in another format, that’s fine too, just send something that I can print and keep. Honesty is super important here! I need to see where you are coming from and what flavors and textures you like so we can make your next steps do-able. The timing and size of your meals is often as important as what they contain. More information is better!

  3. Consent, Policies & Service Menu: Please read and familiarize yourself with how my practice works and how services are priced. I have fine-tuned my policies and services over the years to be as fair and clear to both you and me as possible. Know that for every hour I spend with clients, I spend many more on continuing education or work like evaluating the most honest information about new food products, trendy diets, supplements and legitimate lab testing options. I research the sometimes rare health conditions that my clients may come in with. I read about the latest developments in holistic, alternative, integrative, functional and conventional medicine. Whew, that’s a lot to explain, but I am not a salesperson. I dislike talking about money and love talking about health instead! I will need either a signed copy of my Consent, Policies and Service Menu before we get started or your agreement indicated through my online scheduling system.

  4. Records Release Authorization: As needed, you can download this form to give to your doctor to permit them to share health records with me. Your doctor’s office will typically either send records by fax to my number listed on the form or authorize me to log into a secure practitioner sharing portal and view results. All records are kept strictly confidential.
